Students who need recommendation from me should fill up this form

and send it to me by email or submit a hard-copy.

Please note:

  1. 1.A student can approach me for at most 3 recommendations for summer internships. However he/she should have attended at least one of my courses.

  2. 2.A student can approach me for at most 5 applications for higher studies. However he/she should have done his/her project under my supervision.

  3. 3.Please take prior consent before putting in my name as a referee.
